Medford Library Reserve Request
This form is to be used to place materials on reserve. Please fill out a new form for each class. Each form has space for 5 items, please fill out additional forms as necessary.

Form must be filled out 72 hours prior to materials needing to be available. Materials are not processed on Saturdays or Sundays.

Personal items will be processed with a reserve sticker, barcode and security tape.  Medford Library will try our very best to safe guard your personal items, however, damage and/or loss of property may result from placing items on reserve.

All loan periods under 24 hours (overnight) are library use only.
For personal items, may we adhere a security tape to prevent its unauthorized removal from the library? *
Security strips will not be attached to Audiovisual materials.
