Thank you for your interest in the San Francisco Department of Public Health's 7th Annual Health Impact Assessment (HIA) Practitioner Training Course! The course will be held July 14-17, 2014 in Oakland, CA.

We are committed to ensuring that the training is a good fit for all attendees. As such, based on your responses to the questions below, we may contact you to discuss your application and goals in attending the training. The estimated time for completing this form is 20 minutes. We will send an email confirming whether or not you have been accepted into the course within three weeks of receiving your application.
To ensure diversity of participation, only two people from each organization are allowed to register/participate in the training. If more than two people apply from your organization, we will ask you to decide which two participants should attend. Each person must submit a separate application form. Priority will be given to EARLY registrants.
Registration is open until Monday, June 30th, 2014.
Registration fees must be paid in full by Thursday, July 3rd, 2014.
Upon enrollment, staff will contact you regarding payment options and pre-course readings.
