Matched to #11. I'm not sure why I need to share this with random internet strangers, but I'm having trouble coming to terms with the 10 hard passes that I received on the interview trail. Everyone from advisors to interviewers to A/PDs assured me that I'd match #1/#2 as I was a stereotypical 265+/265+, AOA, straight-H candidate. I know that I'll be happy to train where I matched, but I can't help but shake the knowledge that my specific career plans and niche are probably no longer on the table without the specific opportunities that my #1-#10 offered. Nearest job opportunity for SO is 2 hours away and a huge compromise for them. At the end of the day, though: I Matched. I'm a doctor. I get to work the best job in medicine. I can provide a good life for my family. And even if yesterday's plans are now today's pipe dreams, I still get to have some fun in the ED. So that's me. JAFERD.I'm glad you shared, helps me some. I matched 12 at a program much more prestigious than all but one below that on the list. Everyone is congratulating me, the admin is thrilled. It's like the fuckin twilight zone. I have similar SO issues. I bet your problem was that your 1-10 were just very competitive. My problem I'll never understand. It could have been worse though. And your comments near the end are spot on. This is a great specialty that can be a lot of fun. It is also in flux.

Matched far down my list. I think I'm gonna change specialties. Clearly EM isn't meant for me. I deserve much better than this. < Shouldnt rank a program you dont want to go to. A year wasted, a year of lost physician income, and just this feeling of not being liked by any of the programs I interviewed at.. < When you match next year, we'll be your supportive and fun seniors! There's a home for you in EM!<------I also didn't match into EM last year, felt the same way as you, but it actually made me a stronger applicant. You'll have more experience, you'll have a lot more to talk about during your interviews, and you'll look really impressive compared to 4th years. It really will work out, I met a lot of reapplicants on the interview trail and they were all getting interviews. It feels like the end of the world, but it isn't. You'll get EM just not this year.

When I first asked them for the opportunity to work with them for more exposure, they werent going to write me a SLOE bc it wasnt a month. But by the end of it all, they were more than happy to do it for me. You just have to be a hard worker and show dedication, always be down to do anything, and if they tell you to go early, never leave early. Stay late, basically you should be at about intern level by now, you could really help them out w/those low risk visits. Aka simple migraine w/pt w/known hx of migraines, etc.

How viable is doing an EM fellowship after a FM residency for working in an Emergency department. Asking for a friend. < are EM fellowships a thing? Seems strange considering theres a residency for it < Yeah, heard a lot of FM docs that didn't match go for it. > I heard the EM fellowship is not accredited > But they're still able to get jobs in a lot of places in the southeast and Midwest < theres a big push for most places to only have EM BC/BE docs in their ED. which unfortunatley you wont be with an EM fellowship. it helps make you more comfortable working in rural ED's..but won't lead to working at a level 1 or even level 3 trauma center in a large city.

Do we know how competitve EM was this year? Felt super competitive but the increased in unfilled spots makes me think otherwise. <-- Could be the case of "people hear it's more competitive, people interview at more places, residencies rank the same number of people, but fewer total unique interviews." Given how few spots went unfilled (and we're not sure how many are "truly unfilled") it's hard to say until we see the numbers of people who only applied EM, people who applied as back-up, and people who dual-applied. In addition to MD senior, MD grad, DO, IMG, and other applicant information < same question<13 unfilled, of which most might actually be military, etc., isn't what I'd call a significant number of unfilled spots at all. Since there are ~2300 total, I'd think it fair to say there were pretty much zero unfilled spots < a lot of unfilled military is still a lot. How did it go from 0 unfilled total last year to 13 this year? Military is competitive, and I know someone that was doing EM through military match that didn't get it. < only 2 were military.

For those that didn't match, someone mentioned on SDN that there is a new EM residency opening up in Cali, still pending accreditation but once they have received initial or pre accreditation they will be taking applicants for this year- to start July 2018. It's St. Joseph's medical center in Stockton, Ca. < You mean July 2019? Or are they taking applicants outside the Match? < if they get accredited they'll take applications for residents to start in July. On that note, if you SOAP into a prelim spot and apply to this residency and get accepted what is the consequence? Can you bail on the prelim spot and go to the EM residency? << I don't think so. Like the main match, SOAP match is also a binding contract. Once you accept, youll have to call the program and convince them to release you. Thats a hell of a gamble to give up your SOAP spot for. The website doesn't say they will for sure start it. Only that its pending accredidation. <UCF Ocala Did this last year <--- anyone follow up with the Stockton CA program?----> Anesthesia has a lot of interest since it's the highest paying field of the "less competitive" specialties. Its a binding contract only for 45 days, and I intend to atleast finish out the year until I match EM in 2019. Its not as if I am flaking out on them. Yeah, the hard part is talking with the PD and getting a letter supporting my release, but who knows, if I like anesthesia, I might stay. They do work 50 hours a week instead of 40, but theyre home on the weekends and much less circadian shift. Pay is relatively the same it seems like on an hourly basis, maybe a little less for anesthesia.> Anesthesia is definitely a good field, though the practice is different from EM, pay is similar, hours are good maybe even better than EM. Hopefully it all works out for ya! < i would say hours are 'less' than anesthesia, but they are all over the place. Choose anesthesia or EM, whatever you enjoy doing.<----About the 45 days, dont mention anything at that time, I brought it up in August, but give them enough time to get the letter ready. By the time I needed the letter I had only worked with 2 attendings, but their feedback was positive and the PD said it straight up, said he didnt know me very well given it was the beginning of the year, but that from all my feedback I was a hard worker and I was very upfront and honest about my switch/why I wanted to. Thats really important, you dont want to piss off the PD by trying to hide things. My interviewers said that the letter was very supportive. If you still want to apply you def can, you just should update your PS, and only send the good SLOEs. I didnt even have any SLOEs till November and I didnt do any away rotations bc I didnt expect to love EM as much as I did. I'm sure it will all work out, trust me when I matched I already knew what a huge mistake it was. I was really upset about wasting a year, but it ended up making me a better candidate, youll have a years experience over everyone. Also youll be expected to finish the year, unless you find an open PGY-1 position during the year, which I doubt you will. I was checking for vacancies every sigle day, still no PGY-1 spots. Plus it would be a lot harder for a program to lose an intern vs a PGY-2. <---- I agree. I wasnt going to tell the PD that. I wonder if the PD can tank my app out of spite though. <---The PD can, but I highly doubt he will. The letters going to be written in the first ~1months of you working there, only enough time to get 1-2 attendings feedback. If the PD says you suck, and your other evals are good, its going to look like the PD's doing it intentionally. They wont know you well enough to be able to comment on anything unless you do something really bad. It'll most likely be similar to what the feedback you got from your attendings and residents said. Also, when I asked, I spoke with the cheifs and told them indirectly that I was worried about how the PD didnt know me that well, doesnt have a lot of feedback, how this letter was going to make or break my app. They understood, and they were the ones who actually made sure it was done and submitted on time. Just make sure you're ALWAYS on time, positive, ready to work. Don't treat this like a prelim year, you're categorical and you have all the responsibilities of a categorical intern, even if you're reapplying to EM. And between the time I asked and the time it was due, I had a few more attendings give feedback to the program, and it was all positive. That reinforced that I was doing my job and not slacking. Also, the PD knows you SOAPed into this spot, its not going to be surprising that you want to do EM. Just say you thought you could do it given the similarities (both have procedures, etc) but that you still feel like EM is whats right for you. So I had to SOAP today and fortunately matched into Anesthesia. While I was relieved to have a job, I just felt so depressed man. Sure it beats IM/FM, but part of me wishes that I did a prelim so I could apply into EM next year. I dont want to SOAP again, but EM is what I want to do. Any advice?> it's binding man, there's too much risk involved in asking Anesthesia to release you. Only thing would be to do EM after anesthesia but that sounds unfeasible tbh. I bet it works out even better for you being in anesthesia. such a chiller specialty, you'll be making bank and not stressing at all. < so apparently since anesthesia has a prelim year, you can ask the NRMP for a waiver (automatically granted) to change specialties and you can just reapply to EM next year. The tricky part will be that you will have no SLOE's most likely and no away rotations. Soooo you'll have to schedule an EM rotation if your hospital has it during your prelim year and hopefully you can impress enough to get a LOR. It's tricky but you may be able to do it man. <----I actually matched into a different specialty and realized I wanted to do EM AFTER I matched into a CATEGORICAL position, when I started I had an honest convo with the PD and told him I wanted EM, he released me from the program for the upcoming 2018 year, and I matched into EM this cycle. It def can be done, you just have to talk to the PD, and if you want to change specialties, they cant stop you from doing that. No PD will force you to continue in a specialty you dont want to be in. Across the board its generally accepted, if EM is really what you want, you can reapply, just make sure the PD is ok with it before you start your ERAS app bc you'll have to submit a letter from the PD saying they release you from the program and support you etc. And your stuff is actually saved in ERAS (LOR). I would just get an updated Deans letter. The only risk is if you dont match again, but my program prob would have let me stay if I didn't match, just make sure you're a hard worker. <---- to this previous anesthesia intern, do you have any other advice for me? It seems like I am going to be doing exactly what you did.

All this crying about "interview hoarding" reminds me of a team crying about their opponent running the score up on them. It is what it is. I knew the statistics. But there are 4 programs I applied to on the SOAP, 2 that I had a genuine interest. I interviewed at a different program like 1 hour away and tried to tell them I'd be available for any last min cancelations in January. This same program is on a rank list for someone with 18 interviews, ranked this program low, and they went even though they knew it would be ranked low. So yeah, excuse me for being a little bitter. I'm sure this isnt the case for everyone, but you're not in my position and telling people they didn't deserve to match because they aren't on your level makes you the one with the sense of entitlement. < On the other hand, a program whose interview I almost cancelled blew me away and is now my #4. You just don't always know. I did cancel a ton of interviews at desirable programs that I just couldn't see myself going to, but I'm certainly glad I kept that particular interview. It's a very tough balance. No one wants anyone else to have to SOAP, obviously, but there just aren't enough spots all around. At the end of the day, if the EM spots in the SOAP go to 260/260 AOA Ortho applicants, the blame lies solely on the program leadership for preferring that kind of applicant over the ride-or-die EM applicant with the less shiny application.<people claiming 13 unfilled spots is proof of hoarding? For those that say interview hoarding do not cause anyone to lose spots, yes it does. yeah a single person wouldnt, but you multiple that by an X amount then the interviews do not trickle down. I did not match with 8 ranks, and 4 programs participating in the SOAP I had previously applied to. Two I called in January and they said they were booked full. Hoping for the best in the SOAP but sad to think I wouldnt be in this position if any one of those 4 had interviewed me. < Even if hoarders dropped their invites and you succeeded in matching, doesn't that just mean one less SOAP spot today and therefore someone else won't match? Net is still the same number of people who will match. < Yes,but those SOAP spots may not go to OP, or any of the EM people we know & love. They may all go to ortho bros that failed to match ortho or nsg or IR or whatever.< This < I'm going to be the asshole and say that are multiple opportunities to prove your worth and get those interviews. Step 1. Step 2. GHHS. Research. Leadership. Away rotations. Don't blame your failure on the success of others. < I typed 'this' above. I actually matched. All I'm trying to say is this could have been a lot easier for those who didn't if AOA 260s 270s person didn't go on 24 interviews who will match at their 1 or 2. Just like OP said, all he/she needed was 1 one of those 4 SOAP programs to have 1 more invite (maybe person above 22nd rank) and they would have matched < alternatively, maybe this person could have studied smarter for Step 1 or Step 2, or performed better during their away rotation, or gotten involved in national EM leadership, or put some time into some EM research, or prepped better for the interviews they did get... etc etc... instead of relying on others to let the wealth trickle down. Sure, if "interview hoarders" gave up more interviews, then MAYBE this person would've gotten one. But there were so many more avenues within his locus of control to influence, it seems petulent to whine about "hoarders." <<< AGREED. People who blame hoarders don't understand math. <As someone who went on 16 interviews as a "top" candidate, I kept most of my interviews because I was genuinely interested in checking out all of the programs. I worked hard for this opportunity, and don't feel like I need to apologize for going on more interviews that necessary. Sure, I probably would have matched with half of my interviews, but I'm not going to skip out on the opportunity to interview with a good program if I'm actually considering the program. If that means you don't match, that's your problem. <Look, all people are saying is don't be selfish. you don't seem to be capable of that and that's ok, but don't deny that it's an issue. Just admit you're selfish and don't give a shit about your peers and move on, instead of getting defensive. <-I feel like after that person got 9-10 invites, he probably shouldn't have went to the interview unless it'd be a place they'd seriously want to put in their top 5. "Considering" a program that you aren't super interested in when other people would love that program as a #1-3 is greedy, but you're right, you don't HAVE to be considerate/generous. < Cutting off at 9-10 is silly, 12 is the magic number. <OP, before the interviews I had considered all 16 as viable options for my top 5 spots <So you had 16 high powered programs that gave you interviews that you knew you wanted to go to? Good for you. I think people are mad at people that take interviews at places they have no interest in simply because they had a friend in that area or it was a "vacation". It's within their right to take those interviews, but it's also selfish/wasteful. Another reason why "hoarders" are not worth worrying about: There were about 2300 spots in this match. On average, programs interview 8-10 people per spot. That means there were somewhere on the order of 21,000 interviews conducted this season. There are reportedly 10 spots in the SOAP (NRMP says 13, but that's been inaccurate in the past; last year, it said 6 but there were 0; might include military spots or Saudi spots that somehow didn't fill), so there were maybe 100 interviews conducted for those 10 spots. Let's say those 100 interviews represent 100 distinct applicants who took "one extra interview" than they should have. That's 100 out of 3000 applicants. That's 3.3% of the EM applicant pool, AT MOST, who could be blamed for those 10 spots in the SOAP, and quite likely more like 1% (assuming that the truly intense hoarders took "3 extra interviews.") AND, even if hoarding were the problem, those 10 spots would have gone to AT BEST 10 applicants out of 3000 who applied, so this "problem" only actually affects 0.3% of the applicant pool. anyone hear if medicine prelim -> EM comes with a three year total funding limit that is not the case with surgery prelim?< Your funding is based on your first program that leads to board eligibility. Prelim/TY spots do not count against your funding, in any specialty. If you do one year of a categorical (3+ year program) then that counts against your funding.

My advisor said that one "elite" program gives about 50% of rotators a lower 1/3 or bottom 1/3 SLOE, even if their numerical grade is good. < But even if they got a low rank at an elite progam, the written comments might have been good and said "he'd make a great EM doctor, just not here". Only thing I can think about is maybe they only did interviews at top programs and they all ranked them at the bottom of their lists.<Yeah, could be. ^ Nothing here either. Only 5 interviews, so seriously crossing my fingers...<-- 17 interview invites, 14 interviews, no personal post IV communication. Got a generic one that everyone gets from one IV, but thats about it. < 31 invites, 12 IV, and 5x post IV comm. Before the hate spills, I ended up not liking 2x programs so I knew I was putting them last and 2x I really wanted were last minute openings off a waitlist else I would have cancelled 2x earlier ones. Was shooting for 8x initially. -> side note, there has to be a better system for this, since so many people get their top three I wonder why we can't do two rounds of IV? maybe you get 3x spots/IV Sept-Oct and then they do a round of matching to clear out the top applicants who know where they want to go and that way ther are no hoarders/people taking up spots who know where they wanna go and have good chance of matching there... They put in the hardwork and their SLOE's are the main reason for it, so they earned their interviews. If I remember correctly I got auto-rejected from them as well. <<damnnnn.. so here's a good story, a buddy of mine got rejected from denver early on and then got a secondary email saying "Dear xxxx By now we're sure you recieved our earlier email concerning your application (didn't want to say rejection lols) but upon further review we think you would be a good candidate (shit we got too picky lols) we would like to invite you down for an IV and think you'd be a good fit for our program" hahahaha... saw the email myselfAnybody else regretting a few of their rank orders? Thinking about it now, I would be pretty miserable living in #2 and wish I would have put it as 5 or 6 < how is everyone just thinking of these things now? I feel like it must just be your emotions/your brain playing tricks on you, because you knew the location of this program before you submitted the ROL. Unless some big life event happened to you in the past week i guess . < I think because my #2 is a much better program than my 3 or 4, but in a worse location. I guess I went with strength when deciding to put it there. Also, before certifying I was comparing programs against each other. Now, my mind will go entire days with the idea that I'm 'definitely going to end up at program X'. When given a full day or 2 to imagine my life at only 1 spot, it gives you a little more perspective. It's all good though, I'm not beating myself up about it < ok good, you shouldnt! It sounds like yout 1-4 are all good options for different reasons. Its hard to find one program that checks every box, and youre gonna have to make sacrifices wherever you end up. Do we get officially certified for ACLS for most/all EM residencies?<most expect you to be certified when you show up <--- FALSE<-- no they dont. I had board scores in the 200s/220s got 18 interview offers 1 phone call and 2 rank to match emails. Work your ass off for those SLOEs, kids^other programs require it too, this was just literally the first one I saw after a 15 second search. I didn’t even apply there, but I do know some of the programs I did also require it. Research your programs.

PSA: On Friday, March 9 you will receive an email from the NRMP that states you are eligible for “SOAP”. This does not mean that you did not match. So please – DO NOT PANIC. Can someone please explain to me on the Rank Spot page how there are 98 total #4 ranks, and 102 total #5 ranks? Shouldnt it be a descending tally? <rank spots tab only counts programs that are from the drop-down menu on the original submission form, ie write-ins are not tallied< I think the Rank Spot tab must be missing a program name (or have a program name misspelled compared to how it's labeled on the Rank Lists tab), and that program must appear in the 4th slot multiple times.<-- or some ppl forgot to put a #4 rank bruh.. < Nope, none of the ROLs are missing a #4. You can tell by highlighting any cell in that column and then pressing Ctrl+Down. It skips to the next blank cell... and there aren't any, except from the one person who only posted 3 programs.

hit the gym yesterday for the first time in probably a year. started with phraks greyskull mod workout, very simple bare bones, could barely move the same amount of weight I used to. very sore. excited to hit it again. this also marks day 3 of a 2-week rotation that I am just straight up skipping. no phone calls or emails yet!<< hit the hub for the first time since IV season... dripping buckets for days and my left hand now has carpel tunnel

They should really specify in those emails about SOAP that they are sending them to EVERYONE. I know I'm going to match, but even in my case your mind definitely freaks out briefly when you see an email about SOAP >> Yeah!!! I just got the one they sent out and literally i feel like im having PVCs when i see that initial email pop up.< Don't they know how neurotic we all are? If you really want to address physician wellness, make this whole process easier and stop sending out emails like that!< No SOAP email matters except the confirmation Friday 3/9 and the Match notification on Monday 3/12. So you can disregard any email that aren't those two
