Please complete and submit this registration form. Payment may be mailed, addressed to Melissa Shaw, or dropped off at Lasalle Secondary School, 1545 Kennedy Street, Sudbury ON, P3A 2G1. Please include your business name (cheques are payable to Lasalle Secondary School).

Please note the cost is $50.00 for a spot (8ft). This includes a fire retardant table cloth. Additional tablecloths are available, at a cost of $10.00 each. If you require a table, there is a $10.00 charge. We also ask that you donate an item to our penny table (min. $15.00 value).

Doors will be open at 8:00am-9:30am for set-up, on Saturday, December 7, 2019. When arriving you will be directed to the back of the school to our Receiving Doors. Please have your donation item available upon arrival. Please park at the back of the school, behind the Sudburnia building. We would appreciate if you did not start packing up before 3:30 pm.
