Please complete a form for each person who will attend Smith Stampede. If you have more than one student at Smith, use the oldest student's name and teacher's name. Each name submitted using this form must have previously had their driver's license entered in to the Hall Pass System.  All people who  cannot register ahead of time will need to sign in at the parent check-in table upon arrival at the event.
