Once you are registered for your appointment, here are some helpful reminders to assist in ensuring a safe and smooth vaccine process upon your arrival to the Church.
- Please bring with you an official ID.
- Wear a mask.
- Bringing a copy of your ticket or having it on your phone will assist in ensuring a speedy check-in process. (Not required)
- Please complete the two forms below if you are able to do so. If not we will have forms available on site.
