The goal of the Personal Health Survey is to assist you in determining your risk of certain diseases or illness and to teach you how to lessen the impact of these risks in your life. Because knowledge is power, we hope that by giving you timely information tailored to your specific responses that you will be encouraged to take positive steps to make healthier choices in your life.
Yes, your specific answers will remain completely confidential and will not be shared with your employer, insurance company or any governmental entity. Applied Health Analytics (AHA) values and respects the rights of an individual to keep Personal Information private. Further, AHA values the need to utilize Personal Information that may enable AHA to deliver products and/or services that may lead to the reduction of illness and disease. "Personal Information" is demographic, geographic, personal health and medical information unique to the individual participating in this survey. This information includes, name, address, age, gender, height, weight, family or personal medical history, treatments, lifestyle behaviors, biometric or other type of information. This information is collected on an individual basis based on the participant's interaction with AHA. Aggregate information may be shared with certain entities such as your employer, however, aggregate Information is information in summary form that does not contain any information that would permit the identification of a specific individual. Furthermore, all information will be transmitted over secure networks (https: for instance) and stored in a secure environment as governed by State and Federal law.
Yes, your interaction with the Personal Health Survey and your subsequent participation with any entity providing health services or products is COMPLETELY VOLUNTARY.
Yes, even after you have completed the survey and received your results, you may opt out of the program at any time. Any email that we may send to you will contain an 'unsubscribe' link which will essentially allow you to opt-out of any future communication. You can also call Applied Health Analytics at 615.678.1308 to opt out.
Your physician will not automatically receive a copy of your 'Personal Health Report' directly from us. However, we encourage you to share your results with your doctor or primary care provider and discuss any questions or concerns you may have regarding your results. This is the type of beneficial dialogue that we hope to foster. You may also release your 'Personal Health Report' to your physician by logging into your Personal Health Portal and clicking 'Physician Release'. After taking the survey, your Personal Health Portal may be reached at https://personalhealthportal.net/
Once you have completed the survey, you will have at least one of several options to obtain your Personal Health Report depending upon how your account is structured. Reports may be received via instant download, secure link via email, or via USPS to your address. Moreover, every survey participant will receive access to a secure 'portal' account by which reports and other valuable health information may be retrieved. Special health offers from local and national vendors may also be available to you within your Personal Health Portal. Yes. If you realized that you answered one or more questions incorrectly, you may several options available to change your response(s). If you provided an incorrect name, address, phone number or email address, you may change this information by logging into your Personal Health Portal and changing it there. For all other information changes, please contact Applied Health Analytics at (888) 271-7645.

Usually, not being able to leave the first page occurs because some information is missing. These fields will turn red, and the system will not allow you to continue until you fill in this information. One field that is commonly missed (and does not appear for everyone) is at the very top of the survey. It says "Please select the group that best represents your association:". If you get this question, be sure to pick an association, or you will be unable to leave the page.
