This Back to School form for your new parents to fill out will give you SUCH helpful information so you can hit the ground running! The newly added Getting To Know Your Family page will help you make connections with your students and their families.
This is a zipped (compressed) file containing both a non-editable PDF and an editable Powerpoint file. You will not be able to open either file until you have extracted (aka unzipped) them.
The non-editable file is designed with first graders in mind, but can apply to other grades, as well. You can tweak the editable file using Powerpoint so that it meets your needs.
