ONLINE PRIVACY NOTICE

The data submitted via this form will be kept private and confidential, and the details required are used as follows:

1. E-mail address for forwarding of confirmation message and issuance of OSE Clearance;
2. Student number, name, and college for cross-checking with the CRS if the applicant is undergoing disciplinary proceedings or corrective measures;
3. Gender for ensuring gender equality in terms of the form's accessibility;
4. Purpose of clearance for advising applicants on additional details and/or requirements that may be needed on or before the schedule of release of applicant's clearance, and;
5. Name and student number for printing out the student identification details in the clearance.


INSTRUCTIONS AND GUIDELINES

1) Fill in all the fields in the form;

2)  Effective on 01 July 2021, all applications received until 5:00pm will be processed. OSE Clearance will be issued within the 3 working days upon receipt of the application;

3) Any application received (as per the timestamp) after 5:00pm will be processed the next working day and will be issued within the 3 working days;

4) Applications filed during non-working days (weekends, holidays, declared nonworking days) will be processed the next working day and will be issued within the 3 working days; and

5) The PDF of the OSE Clearance will be issued through the UP Email and/or Alternative Email provided in Online Application Form. Applicants without UP Email please put N/A@up.edu.ph .
