Please complete this form if you are currently registered and receiving accommodations with Disability Services to continue to receive your accommodations for the upcoming semester or Summer Session I and II. A meeting may or may not be required for continuation of accommodation at the discretion of the Coordinator of Disability Support and Success, unless a request to change (adding or removing) current accommodations is requested by the student. This form must be filled out before each semester or Summer Sessions I and II.
Professors/Instructors and other necessary staff will be informed in writing of the approved accommodations, and the student will also receive copies of the accommodation notices. Failure to submit this form may result in a delay of your instructors being notified and accommodations notices cannot be back dated.
