Application Checklist:
1. Parents or guardians should fill in the application form accordingly.
請家長或監護人填妥申請表。　

2. The application fee is HK$75 (it is non-transferrable and non-refundable). Parents or guardians can settle the payment by credit card.
報名費用為港幣75元正(不可轉讓及退還)，家長或監護人可以信用咭付款。

3. Parents or guardians should upload the following documents:：
家長或監護人須上載下列文件：

(a) One recent digitized photo of the applicant.
申請人最近的數碼相片 

(b) Applicant’s Hong Kong Identity Card / Birth Certificate / passport / study visa (in case of non-local applicants, a copy of student’s passport or other travel document with a copy of his/her personal details are also required.)　 
申請人的香港身份證／出世紙／護照／學生簽證（如非本地生申請者，須同時附交詳載其個人身分資料的護照或其他旅遊證件）

(c ) Primary 6 Student Record Form issued by the Education Bureau (if any).
教育局發出的小六學生紀錄表（如有）

(d) Applicant’s school assessment reports in the past two years.
申請人最近兩年的成績表副本

(e) 1-2 mins self-introduction video in English and Putonghua(Optional for Non-Chinese Speaking applicants).
1-2分鐘的自我介紹短片

(f) Certificates of awards, such as art, music, sports, etc (if any).
獎狀證書，例如藝術、音樂、體育等（如有）

(g) Other supporting documents, such as recommendation / reference letter from previous school or other organizations (if any).
其他證明文件，例如由曾就讀學校或其他機構所撰寫的推薦信（如有）

(h) Relevant medical report issued by doctors / psychologists (if any).
由醫生／心理學家發出的相關醫療報告或紀綠（如有）

Personal Information Collection Statement 收集個人資料聲明:

1. All information collected in this form serves to inform the school about the academic and family background of the students and will be used for the purpose of processing the admissions application only.

申請表上所填報的資料，只供校方了解學生家庭、學習狀況以及入學申請考慮之用。

2. Information collected is entirely voluntary and will be treated strictly as confidential. In the event that you do not provide such information, we may not be able to process your application. Applicant information may be transferred between different offices of the School as well as Education Bureau for other administrative purposes.

資料提供與否純屬自願，而有關資料將會保密處理。如申請人未能提供相關資料，本校將未能處理有關入學申請。申請人的資料在有需要情況下將會提供給校內其他部門或教育局作其他行政用途。

3. Should there be any changes of information, applicants can update the information directly in the online application system or submit a request to the admissions office at admissions@css.edu.hk for assistance.

如所提供的資料有任何變更，申請人可直接登入網上報名系統更新個人資料或聯絡入學組(admissions@css.edu.hk)尋求協助。

4. We are committed to protecting the privacy, confidentiality and security of the personal information we hold by complying with the requirements of Personal Data (Privacy) Ordinance with respect to the management of personal information.

我們承諾遵守《個人資料（私隱）條例》下 有關管理個人資料的規定，保障我們所持 有的個人資料的私隱、保密性及安全。

5. I declare that the information provided by me is correct. I understand that this application will be rejected by the school if the information I have provided is false or incomplete.

本人聲明所填報的詳情全屬正確。本人明白如提供虛假資料或不完整資料，校方有權拒絶有關入學申請。
