CWICP services update


The Countess of Chester Hospital's Intermediate Care and Joint Therapies services became the first to transfer under the management of Cheshire West Integrated Care Partnership (CWICP) in December 2018. Joint Therapies consists of Speech and Language Therapists, Occupational Therapists, Physiotherapists and Dieticians working across hospital and community services.

Celebration of Achievement Event
Therapies teams at the Countess of Chester Hospital held a special Celebration of Achievement lunch on May 15th 2019. Long service awards were presented to Carla Fishburn, Penny Brown, Lorraine Such and Kath Swanton. Special mention was also reserved for Sue Fletcher who is in her 38th year with the therapies department. At a Countess of Chester Awards ceremony earlier this year Speech and Language Therapist Helen Bailey collected an unsung hero award and the rapid response falls car, community nutrition service and Bluebell and Poppy Wards received quality improvement and team of the year awards.

Patient feedback
This
presentation slide showcases just some recent feedback received by the therapies teams at both the Countess of Chester and Ellesmere Port Hospitals. The visual was developed by an admin team which processes more than 1,000 appointments every week, supporting Physiotherapy, Speech and Language therapy, Occupational Therapy and Dietetics.

National recognition for community health project

A local community health project has been recognised at a national awards ceremony. The Patient Experience Network National Awards celebrate the delivery of outstanding patient experience by health and care organisations.

An Advanced Community Practitioner Rapid Response Service team managed by Central Cheshire Integrated Care Partnership (CCICP) were finalists in the Integration and Continuity of Care category.

This new service supports adults with acute ill-health or exacerbation of a long-term condition to be treated and cared for at home - where appropriate. It has so far prevented hospital admissions in 72% of cases.

Advanced Community Practitioner Laura Lane said: “The service's success is due to the collaborative working of all community services with primary care. We could not deliver our service without the help and support of the rest of the multi-disciplinary team. We also work closely with hospital colleagues and if we feel patients require a hospital admission this enables a smooth patient journey from community to hospital.”

Community Conversations - What People Want
Led by
Cheshire Community Action on behalf of CWICP, Community Conversations have been taking place across all Cheshire West Care Communities to help us better understand each community's health and care needs. So far, more than 530 people have engaged with the project via face-to-face sessions, one-to-one meetings, over the phone and an online survey. Further Community Conversations are also planned at:
  • Castle Park, Frodsham - Monday 3rd June 2019 10am-1pm
  • Hoole Community Centre - Tuesday 4th June 2019 10am-1pm
  • Upton Village Hall - Tuesday 4th June 2019 2pm-5pm

Care Community  Review - What People Get
To build on the work of Care Community teams, partners need to understand “what works” to help identify areas of best practice and potential options for future working. A CWICP project team has now completed:
  • 11 focus group sessions with approximately 88 stakeholders across Cheshire and Wirral Partnership (CWP), primary care and Cheshire West and Chester Council - with Central Cheshire Integrated Care Partnership (CCICP) sessions to come
  • 14 peer review sessions with 156 staff members across CCICP, Cheshire West and Chester Council and CWP
  • 85 process maps across CCICP, CWP and Cheshire West and Chester Council

This month the team has focused on engaging with local people who use Care Community Team services and presenting emerging themes to the CWICP Board.

The team has met with people from across the system to better understand how Cheshire West's Care Community Teams currently work. A number of workshops are set to take place in June 2019 to provide feedback to those who have been involved and to continue the conversation about how teams could work better in the future.

Care Community Profiles - What People Need
Profiles to outline each Care Community's key health and care issues are being developed by CWICP partners to help with service planning, commissioning and evaluation. Bespoke data packs are being developed to map each Care Community's key health, care and demographic indicators in one place. This will provide a more nuanced understanding of Cheshire West's Care Communities, enable direct comparisons to be made and improve our understanding of the health and care gaps and opportunities. Positive feedback was received from CWICP leaders in April 2019. Next steps include:
  • To develop a narrative to accompany the data to help tell the story of each Care Community
  • To explore the key differences between each Care Community and develop a typical person/family profile to include health, housing and socio-economic status

CWICP public engagement

The CWICP team have been on the road this month at a number of public engagements to help raise awareness of our work and to invite and encourage wider input and support.

Team member Charlotte Love joined forces with Mark Reading from Cheshire Community Action to deliver a presentation at Cheshire West and Chester Council's Adult Social Care and Health Stakeholder Network, held in Northwich.

Kevin Janes from Cheshire Community Action led an input about the Community Conversations project with the chairs of GP practice patient groups across West Cheshire.

The core CWICP team also exhibited at Cheshire West and Chester Council's post-election Borough at Work Marketplace event - making numerous connections with council officers and new and returning elected members.

Thinking and Acting Differently


More than 80 representatives of partners and the public across Cheshire West came together last month to "Think and Act Differently" to transform health and care for our Care Communities. Further Thinking and Acting Differently for Transformation events are planned throughout 2019/20. This video shares some of the early highlights.

Social prescribing in Cheshire West


Social prescribing, sometimes referred to as community referral, is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services. When social prescribing works well, people can be easily referred to link workers from a wide range of local agencies, including general practice, pharmacies, Care Community teams, social care services and third sector organisations.

Brightlife runs four schemes for social prescribing in Cheshire West, in Malpas, Tarporley, Winsford and Chester.  GPs and Healthcare Professionals wishing to make a referral into Brightlife should email referrals@brightlifecheshire.org.uk or call Social Prescribing Manager Christine McMahon on 01606 884442.

News in brief

  • CWICP representatives on the Frail and Elderly programme worked with colleagues in Vale Royal to develop maps of existing services. Connections made by team members at the first Thinking and Acting Differently for Transformation event are now being proactively engaged.
  • A governance workshop was held with system leaders on April 25th 2019, covering ICP fundamentals, national learning and example scenarios for decision-making.
  • Work continues on mapping the current workforce profile within Cheshire West ICP.
