The "Jockey Club Youth Football Development Programme-Summer Scheme 2018" is funded by The Hong Kong Jockey Club Charities Trust, organized by the Hong Kong Football Association, and evaluated by the Department of Sport Science & Physical Education of The Chinese University of Hong Kong (SSPE/CUHK).

Requested by the Hong Kong Jockey Club Charities Trust (funding organization), all participants MUST fill in the questionnaires TWICE. We now invite you to take part in the first evaluation before the programme, the Pretest. Please answer the questions truthfully according to the players' own knowledge. There are no correct or incorrect answers, choose the answers that best reflect your status.

All personal data collected will be kept strictly confidential and will only be used for the purpose of research and analysis by SSPE/CUHK . No personal information will be released or reported.
