THIS IS FOR JACKSON COUNTY HIGH SCHOOL STUDENTS ONLY!

Please complete this form to request your Certificate of School Enrollment/ADAP certificate.

All requests are available for pick up the next day after 1:00 pm in the front office at the new high school.    

Please do not submit multiple requests.

Reminder: Certificate of School Enrollments are only valid for 30 days once dated.
