This form is a request to set up a Spay/Neuter or Wellness Appointment with Virginia Kincheloe Spay/Neuter Clinic. We are a low cost clinic designed to help the needs of the community. We are not a full service veterinarian.

Please be ready to put down a $25 deposit per pet at the time of scheduling. This is a non-refundable deposit that you will forfeit automatically as a donation to the clinic if you don't show or cancel in less than 48 hours.
