Informed Consent

Invitation to Share: We invite you to fill out this survey to help Ottawa food programs do better at meeting your needs. The survey will be open until May 11, 2024.
Benefits to you: When you share your thoughts, it helps us understand your situation and needs. It helps the Ottawa Food Bank know what is working well. It helps us know what we can do better. It helps us make decisions to improve service to you. It also helps us to advocate for change.
Considerations: There is always risk in sharing your personal information. We protect your personal information and keep it confidential, secure and anonymous.
Confidentiality and anonymity: We ask for your name and contact details in the survey so we can give you your gift card. If you share your name and identifying information in the survey, it will be permanently separated from your answers as soon as possible. If you are filling out the survey on an Ottawa Food Bank device, you do not need to share this information in the survey. The Ottawa Food Bank staff will take your contact information separately. Any data shared outside of the food bank will not contain information that could identify you. No names, addresses or contact information will be shared. If you have given your information and no longer want to share it, you can ask to have it removed up until all the surveys are analyzed.
Compensation: We offer a $10 grocery gift card to thank you for 15 minutes of your time. This invitation is open to Ottawa residents only.
Voluntary participation: You do not have to share your personal information. If you choose not to, it will not affect your ability to access food. It will not affect your relationship with the staff or volunteers of the food program you visit. It will not affect your relationship with the Ottawa Food Bank. You can provide your information and still remain anonymous.
How to participate: If you agree to participate, you can fill out the survey online. Ottawa Food Bank staff may be on site at food programs to help or answer questions.
Purpose of data collection: We ask you to fill out this survey to help us and other food bank organizations understand how to offer better service to you.
We use the survey data to improve supports for people using food programs by:
- advocating for government policy changes
- measuring change in community food insecurity
- deciding which foods & how much to purchase / distribute
- deciding which programs are most helpful & which need improvement
- reporting back to funders
- deciding what grants should be offered to food programs
- attracting donors
- making internal strategic decisions
Data sharing: We will share summaries of the data outside of the Ottawa Food Bank network. For example, we might share the proportion of single parents using food banks in Ottawa. Any information that could identify you will not be shared publicly unless we have your permission.
Data storage
Your anonymous personal information and summary data will be stored separately on secure servers. These are located in Canada.
