Milford High School will not be automatically assigning lockers to students as in the past.  This form DOES NOT need to be completed if you do not wish to be assigned a locker for the 2023-2024 school year.

If a student has a need for a locker, then they will need to fill out a locker application form.  This form can also be found on the high school website by clicking "HS Locker Request Form" on the left menu of the webpage.
