Spa Day Request Form 
Hello, thank you for your interest in booking a 2 Hour Massage and Facial Package at En Route Skin Spa.   This package does include a 1 Hour Massage + a 1 Hour Facial for BOTH you and a guest.   Your session will be done side by side in En Route's beautiful location at 1144 Main Ave, Crete NE 68333. So grab a friend, your mom or your significant other and come REFUEL WITH US!

Please read carefully the information below.

 Important! We apologize, but we are not a handicap accessible location. En Route Skin Spa is located upstairs- between Crystal's Bakery & George & Maeve Children's Boutique.  You will need to use the stairs to access their location.  There is no elevator. 

       You will need to fill out ALL the information below.   We are very excited to be able to offer this fun service in our small town, so we appreciate your help with giving us as much information in advance. We are both very busy with our regular booked clients so providing a few dates will be extremely helpful in finding a time that will work for all 4 of us. The more flexible you are the better, but we know you are busy as well. :)   It is helpful if you are able to give us 3 options, that way we will be able to get you in as soon as possible,  BUT IF you only have 2 dates that could work, please just repeat your 2nd preferred date in the 3rd preferred date section.    We can not guarantee any of your date requests will be available, this is just a starting point.  


PLEASE NOTE-  If you are wanting to book an evening time, those slots book up quickly by our clients who prebook. Evening times are available, BUT you may need to book out a ways. 

 You can request Monday-Thursday  (Morning, Afternoon & Evenings) and 
Fridays (Morning & Early Afternoon)
 
This package is NOT available on the weekends.  

A credit card will need to be given to reserve your appointment time, but you will pay each of your providers
 DAY OF YOUR APPOINTMENT- The form of payments accepted-cash, check (which are preferred) or credit card. Normally we have a 24hr cancelation policy but we will request a 48 hour cancelation notice for this group booking.     Click here for details.  Cancelation Policy
