Waupaca County Aging Services Survey

WE WANT TO HEAR FROM YOU! The Aging and Disability Resource Center (ADRC) of Waupaca County is required by the state of Wisconsin to complete a strategic plan that serves as a platform to expand and improve our Aging Program Services. The Aging Programs provide supports to older adults aged 60+ years and includes programs such as the Waupaca County Senior Nutrition Program, Supportive Services for Seniors and Caregivers, Health Promotions and Education, and Benefits Education and Assistance, Transportation, and Dementia Education and Support.

Our staff and advisory committee members participate in the development of this plan, but input and feedback from ALL COMMUNITY MEMBERS is extremely valuable. Please take a few minutes to complete this survey.
