The purpose of this form is to collect the name of the school that your child(ren) attends for the purpose of putting our members in the same school or district in touch with one another. Please enter the name of the school and district that your child attends below. Feel free to fill out another form if your children attend different schools.

This information will only be shared with other LA Chapter FAIR members who attend the same school/district and FAIR chapter leadership. Note that all FAIR chapter leaders have had background checks conducted and been vetted by FAIR national leadership. If you would like your school information deleted/removed at any time, please send an email to <los-angeles@fairforall.org> and ask that your information be removed.
