LTFC, CHRISTMAS SOCCER CAMP REGISTRATION FORM, 2021

Thank you for your interest in this year's Christmas Soccer Camp.

The camp will hold for a week from the 13th of December, 2021 to the 13th of December, 2021. It is open to players (male and female) between the ages of 3-16 years old.  

It focuses on developing the players on the four pillars of football which are: The Technical, Tactical, Physical and Psychosocial Skills.

Above all, the camp promises to be exciting and full of engaging activities to lift the player's interest.

PRIVACY: I verify the date of birth stated above. I fully understand that the player and the parents are bound by the Rules, Regulations, and Policies, including Codes of Conduct of the Club and agree to abide by the Codes of Conduct and all rulings that relate to me.

PHOTOGRAPHY AND VIDEOS: I consent to LTFC photographing and videoing my child for marketing purposes concerning the football club. I acknowledge that if I have concerns regarding pictures or its distribution, I should contact the Club management directly.
