UPDATED: Please complete this form by Monday, April 6th at 6pm. That being said, the earlier you fill it out the better!

A = My team is quite competitive, B = My team is moderately competitive, C = My team is newer/less competitive. Open will have all three division. Female-Matching will most likely only have A and B divisions. Final placements will not be made until early May.

Checks should be made out to "Give Back Ultimate" and sent to Josh Seamon-Ingalls, 960 Westhampton Rd, Florence, MA 01062. Please make sure to write your team's name in the memo field on the check. We do accept credit card payments via PayPal. Send them to sales@ultimategood.org. If you pay via PayPal, the price is $16.25 per person if you pay via PayPal.
