Dear applicant,

by filling in this form you are applying for a training course "Stay Sane" which will happen from 1st to 10th August 2022  in Penzion Bernard, Stará Lesná, Slovakia !
The project is open for participants living in Italy, Germany, Bulgaria, Greece, Slovenia, Latvia, Portugal, Cyprus and Slovakia.

With your application, you commit yourself for the whole project: preparation, participation in the project, dissemination and follow-up activities in your home countries.

We need the following data to prepare everything by the time you arrive. We ask you to give correct and valid data. Some questions are there for you to reflect on yourself and for us to get to know you better beforehand.

Please take your time and thoroughly fill in this application form - the participants will be selected based on their profile and the quality of application forms.
